Looking for a format for weekly special education for each individual

    I will be teaching this coming year 6-8 sped students at different levels. They will be 3-6th grades,  I believe no autism, but B/ED and SLD.  I need a format to perform weekly planning teaching mostly reading and math.  The category may be computer, direct instruction, writing times. spelling time.

    I understand they want me to write a weekly plan for each child.  I could build one from scratch, but if someone has one that works for them, I am open to suggestion or downloads.

    This will be a new style of sped for me and any help would be appreciated.
